Physician Home Care of Utah, was established in September of 2001 by Dr. John Dietlein.   This practice exists for the care of those who are homebound, that is, those who find it difficult or impossible to leave their homes . They often need the help of family, friends or assistive devices (such as walkers or wheelchairs) to leave their home.   Sometimes it is physically unsafe for them to leave their homes.

How Does it Work?
Our office personnel will arrange a time that is convenient for you and your family to meet with Dr. Dietlein who will then take a comprehensive medical history, perform a complete physical examination, and then discuss the diagnoses and treatment plan.  

This treatment plan is individually devised to meet your needs, preferences and may include coordination with other specialists.   Modern technology has now made it possible to bring lab, ekg, x-ray and ultrasound into your home making it unnecessary, in many cases, to go to the hospital to have these services done.

We are privileged to work with many excellent home health care agencies in Salt Lake City to arrange for skilled nursing services such as, wound care, catheter care, IV medications, physical, occupational and speech therapy.    

We provide primary care physician services that include managing chronic illnesses, pain management, prescription needs as well as diagnosis and treatment of new problems.

We are able to assume care as your primary care physician or see you through a temporary condition until you are able to return to your own physician or work with your physician as a consultant.

Charges
Physician Home Care of Utah follows the Medicare fee schedule for physician house calls. After the yearly $110.00 deductible, Medicare pays 80% of those charges and if you have a secondary insurance plan it may pay the remaining 20%.   If the patient does not have additional coverage, they will be sent a statement for the balance.   Our office staff is always happy to assist you with any questions you might have.

Office Hours
Our office is open Monday - Friday from 9am until 4:30pm. After hours and on weekends there is always a physician on call to talk to and answer any questions that you might have.
